There is no form to fill out. When trying to access:

I get a server not found error.
Is there an alternate link to the Video Triage file?

I have reviewed that thread. I tried the link that you referenced in Firefox, Brave, Chrome, IE and Edge. I get the same result each time. Can’t reach the page.
AHA! @fjaviercn Thanks for the link. I believe I have found the issue. My Pihole was blocking it. Once I whitelisted the domain I get the page.
